Hello! We appreciate you joining our waitlist to receive support from one of our Registered Dietitians for nutrition therapy services. While you are on the waitlist, we want to give you the opportunity to obtain documentation of your current nutrition-related diagnoses to help the insurance claim process go as smoothly as possible once we get started on our work together.
To ensure best outcomes and coverage when using your insurance benefits, we request that you have your primary care provider (PCP) or another diagnosing provider (e.g. psychiatrist, mental health therapist, or another specialist) send your nutrition-related diagnoses to us. For your ease, we have provided forms below (separated by health insurance plan) that you can download and send to your provider to be completed. Our fax number and email address for secure messaging can be found on the form.
If you prefer to be the one sending the diagnoses to us instead, please request that your provider send documentation of the diagnoses directly to you. From there, we can send you a form via our electronic health record platform, Kalix, where you can upload the documents yourself to our records.
